Cantilevered Premolar - An alternative restorative approach to molar replacement

 

Alternatively to cantilever the premolar  implant is positioned distally to provide support for a mesial cantilever premolar pontic.

 

 

 

Ideally the molar implant restoration should be positioned in the centre of the edentulous space but if a standard 3.75mm diameter implant results in a  grossly over contoured crown.

Large mesiodistal space and narrow residual ridge often limit implant diameter and restrict implant positioning.

 

 

 

 

 

 

 

 

 

Indications

 

 

 

  1. remaining dentition sound, stable occlusion
  2. mesial distal space 11-14mm.

     

  3. long axis of the implant 90 degrees to the occlusal plane

     

Transmuosal Approcah (single stage)

 

Combi implants deliver excellent primary stability sufficient to permit the placement of a healing abutment on implant insertion. After three months soft tissue healing is complete and impressions can be taken.

 

 

 

A friction fit abutment does not require a screw, eliminating micro leakage associated with the micro gap. The crown is made from a composite restorative material bonded directly to the friction abutment.

 

 

 

 

Premolarised posterior occlusion has a narrow occlusal table with low cusp angles reducing lateral load.

The cantilever premolar is very amenable to routine oral hygiene procedures and well tolerated by patients
